Nazareth Housing is proud to partner with Public Health Solution’s WholeYouNYC network to connect New Yorkers enrolled in Medicaid with essential social health supports — including housing, food, and transportation — at no cost.
These supports are critical to preventing homelessness and promoting long-term stability, which is at the heart of Nazareth Housing’s mission. When families have access to nutrition, healthcare-related transportation, and safe housing, they are better able to remain stably housed and move toward economic security.
Through this partnership, individuals and families can complete a short online form to be connected with nutrition assistance, housing support, transportation, and other vital resources that strengthen health and housing outcomes.
